A case of tension gastrothorax also emphasizes this point. Tension Gastrothoraz may be confused with a tension pneumothorax. There is hemodynamic compromise, tracheal & mediastinal deviation, and decreased air entry in the affected hemithorax (usually left) - see x-ray below. Tension gastrothorax occurs in spontaneously breathing patients with a large diaphragmatic tear (usually blunt trauma).
